What We Do

CHP’s goal is to help educate people about their own health, risks and care options so they become better advocates for themselves. To accomplish our goal, we work with community, medical and religious stakeholders to reach out to the diverse neighborhoods within Ward Four.

CHP’s programs and events aim to:

  • Raise community awareness about racial and ethnic inequities in healthcare.
  • Increase access for target population to up-to-date information on health risk behaviors and chronic diseases.
  • Link people to preventive healthcare services and early detection, screening, and testing.
  • Link people to accessible and appropriate healthcare providers.
  • Recruit community focus groups to help with development of culturally relevant interventions tailored to our needs.
  • Increase visibility of existing healthcare programs and social services.
  • Enhance quality of life.

The Kiosk

A centerpiece of CHP's activities is to establish a network of touchscreen interactive kiosks that deliver reliable health information tailored to the specific healthcare needs of racial and ethnic populations. The program focus is use of technology to facilitate access to evidence-based risk assessment and disease prevention information. The kiosk program was inspired by early meetings with community stakeholders held in 2003. Our touchscreen kiosks will put basic health knowledge at the fingertips of Ward Four residents in convenient neighborhood locations. The kiosks provide printed copies of risks assessments for users to share with their physicians; and a direct dial telephone to partnering healthcare organizations.

Your Disease Risk is the first module for CHP's kiosk project. It is an interactive risk assessment and prevention tool developed at the Harvard Center for Cancer Prevention for five of the most important diseases in the United States such as cancer, diabetes, heart disease, and stroke. These chronic diseases are among the leading causes of death in Ward Four.

Your Disease Risk collects the latest scientific evidence on disease risk factors into one easy-to-use tool. With generous support from Harvard University and the Siteman Cancer Center, CHP has been enabled to use this program on our public kiosks.

CHP’s ongoing efforts to implement our kiosk program have been rewarded.  With lots of community support, CHP successfully the raised necessary funding to develop the touchscreen kiosk softwareHEALTHY? Find Out!, the new program launched in Ward Four during 2011.

Focus Groups

In 2009, CHP partnered with Westat to recruit (30) English and Spanish-speaking women for focus groups. The purpose was to develop culturally appropriate educational materials to launch an Ovarian Cancer Education Campaign in Ward Four.

CHP would like to congratulate the women of Ward Four who participated in these focus groups. Thank you for sharing your time, opinions and life experiences about women’s health issues.

The outcome of this collaboration is a bilingual brochure entitled, IS YOUR BODY TALKING? Take Time to Listen. This project was funded by District of Columbia Department of Health and DC Cancer Consortium.

The Market

A popular neighborhood event is CHP’s Antiques & Ole Stuff markets. CHP markets are held monthly on Saturdays from May thru September. Market hours are 8:00 a.m. - 3:00 p.m.

Look for us at the corner of 13th and Peabody Streets weather permitting.

Since 2005, CHP has organized monthly outdoor flea markets to facilitate community outreach and enhance quality of life. The markets create a wonderful opportunity to meet & greet neighbors and to disseminate bilingual health information. Market visitors are from diverse ethnic backgrounds and many lack access to a regular health care provider. CHP maintains a free bilingual health information table for market visitors.

In 2009, CHP began to partner with medical mobile health units to provide market visitors with free health screenings. To facilitate access to care in Ward Four, only mobile units with bilingual health technicians are invited to participate at CHP markets.

ON THE ROAD TO ELIMINATING HEALTH DISPARITIES: Ward by Ward

In 2008, CHP sponsored our first community wide health issues forum held at DC Convention Center. The purpose of this forum was to facilitate a lively community discussion on challenges and factors that may contribute to health disparities from different perspectives of patients and residents, physicians and health insurance providers.

The keynote speaker for this full-day event was Dr. Gloria A. Wilder, President and CEO of CORE Health. Dr. Wilder is a nationally recognized speaker on social determinants of health and economic segregation.

The morning session provided participants with an overview of chronic diseases in DC; challenges faced by physicians and health insurance providers; conflicts between health literacy and patient’s rights and; and how these challenges contribute to health disparities. The Festival

The Health Festival is a large indoor/outdoor community event with live music on the street. Street banners, radio and TV announcements promote the Festival for several months prior to Festival Day. Our guests are neighborhood residents and visitors from across the city. The Festival is free and open to the public.

Area hospitals, universities and other community partners join with us to provide visitors with free health-screening and referral services for cancer, diabetes, high blood pressure, heart disease, HIV/AIDS and other sexually transmitted diseases.

Partners also provide free computer training to teach visitors how to find evidence-based health information on the Internet. Bilingual information on health concerns such as breast cancer, childhood asthma, stroke, smoking and other critical health issues. Physicians conduct bilingual workshops on risky health behavior, disease prevention and healthy lifestyles. Visitors love the great door prizes, sports club passes, games for children and healthy meals.
