

Community Health Partnership (CHP), Inc. is a unique, grassroots organization working in the District of Columbia's
Ward Four community to develop community-driven solutions to one of our neighborhood's -- and our nation's -- biggest challenges: eliminating inequities in healthcare across racial, ethnic and socio-economic groups. Our goal is to help people become better self-advocates for their own health.

CHP's mission is to build healthy neighborhoods through education and community outreach activities that aim to improve health literacy, link people to care and treatment, and enhance quality of life for all members of our community.
We operate within a street-level environment that brings together diverse groups, non-traditional partners and social networks with experts in a variety of health-related fields, consituencies, and social service organizations. We value neighborhood support and work to build relationships and establish trust with our community partners.

HEALTHY? Find Out! collects the latest information on health topics into one easy-to-use tool called a kiosk. You can use CHP kiosks to find out if you are at risk for conditions such as cancer, diabetes, heart disease, osteoporosis and stroke. CHP health kiosks locations are:
